ViaTerm and ViaPro
30 provide term life insurance to age 85. The death benefit is level.
Premiums are guaranteed level for the first 10, 15, 20, 25 (ViaTerm) or
30 (ViaPro) years. Rates increase annually thereafter, subject to a guaranteed
maximum. With ViaPro 30, the client receives a cash refund payment if
the policy is terminated or fully converted in years 7 through 25.

Fee
A $100 policy fee (non-commissionable) is charged each year.
Underwriting
Classes
Class 1 - Preferred non-tobacco user - Applicants who have not
used tobacco in any form in the past two years and meet the "preferred"
criteria requirements for this class.
Class 2 - Standard
non-tobacco user - Applicants who have not used tobacco in any form
in the past 12 months and do not meet the "preferred" criteria. This class
includes substandard risks for the non-tobacco user classes, including
no nicotine in the urine.
Class 3 - Preferred
tobacco user - Applicants who use tobacco in any form and meet the
"preferred" criteria for this product.
Class 4 - Standard
tobacco user - Applicants who do not qualify for any of the other
classes. This includes substandard risks for the tobacco user classes.
Special Features
ViaPro
30 Premium Refund Option
ViaPro 30
automatically includes a premium Refund Option (PRO). The policyowner
receives a cash refund payment if the ViaPro 30 policy is terminated or
fully converted in years 7 through 25. If termination or conversion occurs
between years 7 and 8, the policyowner will receive a portion of the year
8 refund. The cash refund payment will be based on the face amount, age,
sex, rate class and length of time the policy has been in force. Additional
premium for riders, table ratings or flat extras will not affect the amount
of the cash payment.
Convertibility
The policy
is convertible until the insured’s 70th birthday or the 10th policy anniversary,
whichever is earlier. The client can convert to any permanent plan we
make available for conversion. For ViaPro 30, the cash payment, if any,
from the Premium Refund Option may be applied to the premium due for the
converted policy. ViaPro 30 is convertible to the 15th policy anniversary.

Accidental
Death Benefit Rider (ADB)
The Accidental Death Benefit Rider provides protection if death occurs
within 365 days after an accident. Death must be the result of injuries
sustained in the accident. The minimum issue amount is $100,000. The maximum
issue amount is the lesser of twice the base policy face amount or $300,000.
The combined accidental death benefit for both the ADB and ADD riders
cannot exceed $300,000 or two times the face amount, whichever is less.
The rider is available at issue only. Premiums are level and payable until
the rider expires at the policy anniversary nearest the insured's 65 th
birthday.

Waiver
of Premium Rider
The Waiver of
Premium Rider provides that, in the event of total disability, we will
waive an amount equal to the premium for the base plan, the Accidental
Death Benefit Rider and the Accidental Death and Dismemberment Rider.
There is a $3,000,000 maximum limit for the Waiver of Premium Rider. The
rider is available at issue only. Issue ages are 18-55. Premium for this
rider remains level during the initial level period of the base policy
and increases annually thereafter.
If total disability
begins before age 55, premiums will be waived as long as the disability
lasts. If total disability begins between ages 55 and 60, premiums will
be waived for the greater of two years or to age 60. The rider expires
at the rider anniversary nearest the insured’s 60th birthday.

Accidental
Death and Dismemberment Rider (ADD)
The Accidental Death and Dismemberment Rider provides protection if death
or dismemberment occurs within 365 days after an accident. Death or dismemberment
must be the result of injuries sustained in the accident.
The rider pays the
following percentage of the rider benefit if the insured dies or suffers
total loss from these disabilities:
100% Death
100% On the total loss
of :
- sight of both
eyes,
- both arms or both
hands,
- both legs or both
feet, or
- one hand and one
foot
50% On the total loss
of:
- sight of one eye,
- complete hearing
of both ears,
- one arm or one
hand, or
- one leg or one
foot
25% On the total loss
of:
- the thumb and
index finger of one hand, or
- the complete hearing
in one ear
100% For
an accident not provided above which will cause the insured:
- total loss or
weakening of his/her physical or intellectual strength AND
- the loss of at
least 80% of his/her labor capacity
Total benefits resulting
from one or more accidents may not exceed 100% of the benefit.

amount is $50,000. Maximum issue amount is the lesser of twice the base
policy face amount or $300,000. The combined accidental death benefit
for both the ADB and ADD riders cannot exceed $300,000. Premiums are level

Billing
procedure
A client can choose among four premium payment modes: annual, semi-annual,
quarterly or monthly pre-authorized premium payments.
If the pre-authorized
monthly premium payment method is selected, premiums are deducted automatically
from the policyowner’s checking account. The checking account must be
with a U.S. bank located in the U.S. Withdrawals will be made in U.S.
dollars. To implement a pre-authorized payment plan, your client must
submit two monthly premium payments, a voided check and the pre-authorized
payment form with the Personal Fact Sheet.
Full and current premiums
must be submitted before a contract will be issued.
The rates in the
premium table are for annual billing – the most economical mode. Use the
factors in the following table to calculate the premiums for other modes.
